Nominate a Family or Individual

Thank you for taking the time to nominate an individual for potential support from the JP Strong Hope Foundation. We are dedicated to empowering individuals with disabilities to live more independently and comfortably through home modifications and other accessibility solutions in accordance with the Americans with Disabilities Act (ADA).

Eligibility Criteria

  • SCI Diagnosis: The nominee must have a medically confirmed spinal cord injury (traumatic or non-traumatic).

  • Location: The nominee must reside in the Hampton Roads area.

  • Demonstrated Need: There must be a clear and demonstrated financial, medical, or social need arising as a result of the SCI.

  • Consent: The individual being nominated (or their legal guardian) must be aware of the nomination and consent to it.

  • Support Type: Support may include (but is not limited to):

    -Mobility equipment (wheelchairs, lifts, etc.)

    -Home or vehicle modifications

    -Rehabilitation or therapy services

    -Emergency financial assistance

    -Peer support or community integration

Review Process

After submitting this nomination:

  • The JP Strong Hope Foundation's review committee will carefully consider all nominations.

  • We may contact the nominator and/or the nominee for further information or clarification.

  • Due to the volume of requests and available resources, we cannot guarantee that all nominees will receive funding.

  • Nominees selected for support will be contacted directly with information about the next steps.
